△Personal Protective Equipment against SARS during Tracheal Intubation: CGMH-Kaohsiung Experience

Aim of investigation: To report our extraordinary experience of using subsituted personal protective equipment during intubating SARS patients.
Methods: Retrospective analysis of impact of PPE on the incidence of SARS of HCWs after they performed tracheal intubation during SARS outbreak.
Results: From May 1 to May 31, we accomplished 62 tracheal intubations on SARS (suspect and probable) and non-SARS (who knows!) patients in CGMH-Kaohsiung. None of us (8 anesthesiologists and 23 nurse anesthetists) contracted SARS although no PAPR or Stryker T4 had been applied. Instead, a simple but effective home-made PPE proved to be safe. In contrast, 12 HCWs (internists, respiratory therapists and nurses) got infected when they intubated SARS patients without high-level protection. Chang-Gung spirit (hardworking and simplicity) seems to be more effective than vaccines against SARS.
Conclusion: PAPRs and Stryker T4 are not the exclusive PPE against SARS. Some alternatives do exist.
麻醉醫師替SARS病患進行氣管插管時的保護裝備:高雄長庚經驗

研究目的:本篇論文是報告麻醉同仁替SARS病患進行氣管插管時的保護裝備之高雄長庚經驗。
方法:回溯性分析醫護人員與麻醉同仁穿著個人防護性裝備與否,因替SARS病患氣管插管而感染SARS的機會。
結果:從92年5月1日至5月31日止,本科8位麻醉專科醫師與23位麻醉專科護理師總共出勤62次,替本院所有SARS病患(suspect及probable)與非SARS病患(包括病況不明者)進行氣管插管任務。當時所有本科同仁皆未穿著正壓濾進氣流防護裝備,僅著簡易克難型套裝。與12位本院其他科之同仁因參與SARS病患氣管插管而導致感染慘狀相比,本科同仁全部安然返航。此意謂長庚「勤勞樸實」之院訓,足以對抗陰險的SARS。
討論:若PAPRs與Stryker-T4的個人保護裝備可作為SARS氣管插管時的標準配備,但並非唯一的選項。(感恩!)
△Abnormal automaticity and triggered activity in canine pulmonary veins sleeves

Aim of investigation: To clarify the mysterious story regarding PV arrhythmogenesis proposed by Chen et al. (CVR 2000; Cric 2001; JACC 2002; BRC 2002; Cric 2002).
Methods: Cellular electrophysiological study of canine PV sleeves was performed using conventional microelectrode intracellular recording technique.
Results: In PV sleeves from 100 Mongrel dogs, all the action potentials were fast response. No spontaneous pacemaker activity or depressed slow response action potentials were observed. Abnormal automaticity and triggered activity were reversibly induced by various modalities, such as potassium channel blockers, hypoxia, acidosis, hyperkalemia and intracellular calcium modulation. Transient outward potassium channel blocker did not elicit EAD or triggered activity.
Conclusion: In addition to our recent publication (Int J Cardiol 2003), this report further gave crucial contradictory evidence against Chen's scandalous story.
狗肺靜脈之異常自動節率與促發活性之研究

研究目的:本篇論文是探討國際著名的陳氏研究群連續發表有關肺靜脈心電生理特性之神秘黑洞(CVR 2000; Cric 2001; JACC 2002; BRC 2002; Cric 2002)。
方法:採用傳統細胞電生理技術及國人愛吃的土狗。
結果:在100隻國產土狗之肺靜脈中,從未發現異常慢速心肌動作電位。如以標準化之模式則可以誘發異常自動節律與促發活性。包括酸性環境、缺氧、鉀離子阻斷劑、高鉀、及胞內鈣離子增減等。例如短暫外流鉀離子流阻斷劑並未引發EAD或促發活性。
討論:除了吾人最近剛發表的論文外(Int J Cardiol 2003),本篇實驗結果再度對台北榮總的陳氏研究群發表有關肺靜脈心電研究論文,提出一系列之挑戰!唯獨本篇論文所屬之研究計畫申請案,今年度已遭國科會委任之某些麻醉初、複審所阻撓!
